To apply for a U.S. passport, download the appropriate passport package. The package has all the documents you need for each type of passport.
New U.S. Minor Passports
- Be under the age of 16 years old
- Have the permission from either the parent(s) or legal guardian(s)
- Be able to appear in person with your parent(s) or legal guardian(s) at your local Acceptance Agency.
Name Change Passport
- If you have changed your legal name since your last passport was issued;
- and the passport’s issue date is within one (1) year;
- otherwise renew your passport.
Secondary Passport
- Issued at the discretion of U.S. Department of State to frequent travelers that do not have time to acquire visas between trips;
- or for security reasons.
New U.S. Adult Passport
- If you are a first time passport applicant;
- Your passport was issued before the age of 15;
- Your passport was issued more than 15 years ago;
- Your expired passport is not in your possession;
- Your current valid passport is damaged.

U.S. Passport Renewal
- You already have a passport that is not damaged;
- & your passport was issued within the last 15 years;
- & you were over 16 at the time it was issued;
- & you have not changed your legal name;
- or you changed your legal name & can prove it.
Lost Or Stolen Passport
- Apply to replace your lost or stolen passport only if the missing passport is still valid.
- If the missing passport has expired, apply for a new passport.
